Scenario:
You are a nurse at City Mental Health Clinic. Your patient has anxiety and depression.
Case Notes:
Patient: Mr. John Wallace
Age: 41
Marital Status: Divorced, lives alone
Occupation: Accountant
Symptoms:
• Persistent low mood, fatigue, sleep issues (6+ months)
• Poor concentration, work performance affected
• Occasional panic attacks (triggers: crowds, deadlines)
Medical History:
• Hypertension (on amlodipine)
• No previous psychiatric illness
Treatment:
• 6 sessions of Cognitive Behavioural Therapy (CBT) – limited progress
• Prescribed Sertraline 50mg daily – started 2 weeks ago
• No suicidal ideation reported
• Alcohol: 2–3 drinks/week
Social: No close family or support network
