Reason for Admission: Fall at home – minor head injury, bruised left arm
Medical Background:
Osteoporosis
Poor vision (cataracts – not yet operated)
Mild dementia (diagnosed 2023)
Lives alone; occasional help from niece
Hospital Management:
Head CT: normal
Observation for 24 hours – no complications
No fractures
Discharged 21 March with mobility aid (walker)
Recommendations:
Daily safety checks at home
Monitor for signs of head trauma (vomiting, confusion)
Referral to falls prevention program
Home modification (install handrails, remove rugs)
Niece to be involved more regularly
Cognitive review advised
